Thank you for reaching out with your question. The reason why you are seeing the "Created by" field on this form is because this is a default form, where all of the fields are displayed. You have the ability to create custom forms in TrackVia, where you can specify which fields you want to display, the order of the fields, assign different names to your fields, as well as format your custom forms with colors.

Once the custom form is created, it is important to edit your dashboard that you want to add this form to, and "swap out" your default form currently being displayed on the dashboard for the newly-created custom form.
