From our call earlier, it sounds like you are looking to add a widget that would allow your Organization's logo to be added onto a Form, to then be saved as a PDF and printed with the company logo embedded onto the page. At this time, you would be able to add an image to a Record, but not embed it in a Form, therefore, this would be an excellent suggestion for an enhancement. I went ahead and logged a feature request in your Organization's name with our Product team for them to review and consider implementing this functionality in a future Project. I have added the email address you have provided during our call to that request as well, once this feature is released to the system, you will be notified.
In the meantime, another option for generating custom PDF documents and being able to include a logo on them as well would be utilizing Formstack Documents (WebMegre) Microservice with TrackVia. Through Formstack Documents, you would be able to create a custom-formatted template that would allow generating custom PDF or MS Word documents and including select fields in these documents in a desired format and layout.
